This level of care is for those needing a short rehabilitation or nursing stay to transition back to their prior level of care or independent living. Most of the patients seeking this care have experienced surgery, a stroke, or some other sort of debilitating setback. Their physician expects them to make a recovery and return to independent living or their living situation before the incident.

The Rehabilitation Program Designed for You

The Rehabilitation Program at Williston Care Center is a specialized program that works with patients up to 7 days a week to help them get back home, to work, independent living or what else their goal is. We are dedicated to improving the strength, mobility, and independence of our short-term rehab guests so they can return to their daily lives as quickly as possible.

Our certified therapists will use several different techniques on different equipment that work your body, mind, and coordination. Our modern rehabilitation gym includes state-of-the-art equipment and an ADL Suite (Activities of Daily Living). We work to treat your current physical state as well as get you ready to go back home.

The customized therapy plan may include one or more of the following therapies:

Often with short-term therapy there will be specific benchmarks and milestones we are looking to help you accomplish. Continuing After Short-Term Care

Once you are ready to go home, we can help with coordinating any continuing care you may need. While you may have reached the milestones your physician set for you, trauma to the body can take a while to heal.

Occasionally residents and families may choose to remain for long-term care. This is a decision that may come from deciding what is best for a patient. My mom was admitted here for rehab after being in the hospital for a few months. The first day was really rocky, but it was her first day, which can be expected. The therapy department was outstanding. Natalie and Charlie are the best! I had a few questions about her medications and other concerns - the charge nurse Meredith was extremely helpful in addressing our concerns. Brie and Diana, the two social workers are amazing! They checked on my mom almost daily and made sure to keep us involved in her care almost daily! May God bless them!

God bless the staff who work at Williston Care Center! My mom has been here almost a year on long-term care. She's bed ridden, has moderate dementia, but has received good care overall, and has had a good recovery from Covid. I'm happy her basic needs are being met, and they help her use her phone to call me. She hasn't had any problems with theft of items. The turnover among the caregivers seems high, and their case loads are high, but staffing seems to be a problem at most nursing facilities right now. I visit her weekly and the staff are always helpful when I ask. Audrey always offer a smile at the front desk and asks about my mom. The facilities (paint, furniture) and resources (beds, sheets, blankets, etc) could use some updating. After a horrible experience at another facility, I'm grateful for the people who work at Williston Care Center!

My husband was admitted for short term rehab and throughout his stay I visited daily. All departments were helpful when needed. Direct care care were great as well. The overall environment is cheerful and happy. Everyone was very welcoming. My husband was very pleased with the food as well. Overall it was a wonderful place for him and I would recommend it for your loved one if and when needed.

Why should I go through rehabilitation at a nursing home?

We have the full rehabilitation set-up including equipment, certified therapists, and a great location. Just like we provide a comprehensive home-like atmosphere for our residents, we do the same for our short-term rehabilitation patients. You will get the great dining, relaxing grounds and courtyards, and 24-hour medical access along with everything else you need for your rehabilitation. Complete rehabilitation requires your therapy sessions as well as relaxing downtime in order to heal.

 

How is a nursing home different from a hospital?

Nursing homes in the past used to be stark, institutional places that were cold and lonely. We cannot stress enough how much nursing homes have changed.

  • Our centers are designed to provide the home-like atmosphere you want for your loved one while still providing any medical care they need.
  • We provide social activities for mind and body well-being, companionship, and socialization.
  • Our dining services are specialized and provided in a posh and comforting dining room.
  • We do not restrict visiting except in certain health crisis situations.
  • We have rehabilitation services for extended care and comfort.
  • Residents can bring personal items with them.
  • We celebrate holidays, birthdays and promote a sense of community.
What safety measures are in place?
Safety measures are one of the reasons families may decide their loved ones are better situated at a nursing home than in their own homes. We have 24-hour nursing and medical staff, medical supplies and equipment, medication and are also designed with fall and safety precautions in mind. We also make sure your loved one is under constant surveillance while still respecting privacy. Locked security systems, cameras, visitor logs, supervised outings, emergency call buttons and more are designed to keep safety and security in place even while in a home-like atmosphere.
What can my loved one bring with them?
We proved all the furniture your loved one needs. We do recommend bringing pictures, a favorite blanket, or cherished knickknacks. We discourage bringing anything of value, electrical items or anything that takes up large amounts of space. When you come for your tour you will get a better idea of what space is available and we can provide you with some suggestions as well.
